Required Forms & Documents


We have worked super hard to make your life just a little bit easier this summer! RMC Youth Ministry and the state of Colorado take your child’s safety incredibly seriously. Unfortunately, that often means…paperwork.

Please note that all of the required documents and forms can be found in the online account that you use to register your camper. Forms can be completed and signed electronically while documents [5 of them] need to be downloaded, completed with pen and paper, & then uploaded to your account. The online forms should be completed at the time of registration. The health history form is included and must be completed at least 10-days prior to your camper’s session start date. The 5 documents (with instructions) are available in your online account, as a PDF attachment in your camp registration confirmation email, and at the links provided below.

Remember, Check-In Day will go much smoother for you and the other families if you have completed all of the tasks (forms and documents) on your account before arriving at camp.

Priority Check-In will be available for families who have paid in full, completed all of the electronic forms, and uploaded completed, legible scans of the 5 required documents at least 48 hours before the start of their camper’s session.

Families who wish to bring physical, hard copy documents to Check-In day may still do so. Please remember to bring a complete packet with all 5 documents. Legally, we cannot keep any campers under our care who have even 1 incomplete document.
